Client Rights

I understand my basic rights as a client. These rights include:

– the right to be treated with dignity and respect

– the right to expect professional and competent help

– the right to ask questions about anything related to my treatment

– the right to know information concerning diagnosis and treatment

– the right to participate in decisions related to my treatment

– the right to request another counselor or agency

– the right to end treatment at any time

I understand that if I have a complaint/grievance, I should:

– write a brief statement or description of the violation

– include your name and contact information
